Summary

We are seeking a part-time (average 5 hours/week) Certified Diabetes Educator-RN in the Baylor College of Medicine (BCM) / Texas Children’s Hospital (TCH) Division of Pediatric Diabetes & Endocrinology for a quality improvement project in pediatric diabetes care. The Certified Diabetes Educator position will be contributing to a diabetes care innovation project.

 

The Certified Diabetes Educator-RN will monitor and report on key project related clinical metrics, maintain the diabetes registry, identify, and prioritize outreach efforts. S/he will work in close coordination with the pediatric diabetes care team under the supervision of the Principal Investigators. The successful candidate will work with the principal investigators and project team to carry out the quality improvement project. 

 

Project activities may take place at TCH/BCM locations in the Houston metro area: Texas Medical Center, West Campus (Katy), Woodland Campus, Cy-Fair Health Center (Cypress), Clear Lake Health Center (Webster) and Sugar Land Health Center. The candidate must be willing to travel to those locations. Mileage for travel and parking expenses may be reimbursed. 

 

The successful candidate will be trained in skills and information relevant to pediatric diabetes, including: 

  • Learning about pediatric diabetes by observing patient/family diabetes education classes and/or medical visits at the Texas Children’s Hospital (TCH) Diabetes Care Center, and completing readings and tutorials with the Principal Investigators and other diabetes experts at TCH. 
  • Learning to use the TCH electronic medical record, EPIC, to identify and track patients with diabetes.
  • Learning to collect and manage data from quantitative questionnaires, diabetes-related technologies such as blood glucose meters, insulin pumps, and continuous glucose monitors and electronic medical records.
  • Learning to work independently and as part of a larger study team.

Job Duties

  • Assesses, develops, implements, and monitors a comprehensive plan of care through an interdisciplinary team process in conjunction with the patient/family.
  • Partners with the diabetes care team to allocate resources associated with the patient’s risk assessment.
  • Assists with the patient/family in coping with diagnosis by optimizing their self-care knowledge and abilities.
  • Performs comprehensive goal-focused and personalized assessments with tailored plans for self-management education and care.
  • Supports onboarding and sustainability of diabetes technology including insulin pump and CGM and supports families in uploading data for remote care.
  • Collaborates with community health education/outreach organizations and providers to promote health for youth with diabetes.
  • Prepares and monitors outcome data to assist in identification of improvement opportunities.
  • Utilizes quality improvement process to identify improvement opportunities and evaluate programs and services and their outcomes.
  • Takes evening and weekend telephone calls on occasion, as needed.
  • Attends and participates actively in team meetings approximately one day/week with virtual options available.

Minimum Qualifications

  • Associate's Degree or Bachelor's Degree in Nursing.
  • Four years of relevant experience as a licensed Registered Nurse (RN). Degrees in lieu of experience will not be accepted.
  • Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing. Certified as a Diabetes educator (CDE), Basic Life Support (BLS) Certification.

Preferred Qualifications

  • Minimum of 1-year experience in diabetes care and education.
  • Previous experience interacting with patients and families and/or experience in health care settings.
  • Experience with culturally diverse children and families.
