Job Description

Provides home based care that supports optimal health and wellness for the patient, within the scope of practice of a paramedic (EMT-P), under the direction/supervision of a Licensed Independent Provider (LIP) and appropriate health team members.
Performs assessments including those for high risk patients and reports findings of both in-depth and focused healthcare assessments, screenings and examinations to the appropriate team member (s) (this may be done using tele-presence, telephone and/or written documents). These assessments take place in a residential environment requiring the caregiver to operate a motor vehicle and practice in a variety of community settings and locations.
Community Paramedics support existing health services in partnership with other health professionals including community organizations. When appropriate, Basic Cardiac and Advanced Life Support may be provided as outlined in existing Cleveland Clinic EMS protocols in coordination with local Emergency Medical Service providers.
Quality assurance is an integral part of the hiring and onboarding process of new paramedics, including orientation, training, coaching, chart and documentation reviews, peer and annual performance reviews.
Manages implementation and maintenance of designed programs. Plans, develops and maintains systems and procedures. Liaison between department and other departments and outside communities.

Job Responsibilities:

  • Examines, screens, treats and coordinates health services for patients (may include point of care laboratory testing based on paramedic’s education, training and competency).
  • Conducts/assists with  post-hospital care including, but not limited to,  home safety/living condition evaluations, medication adherence/education, dressing changes, checking of vital signs and reinforcement of post-discharge/post-office visit education.
  • Observes, records and reports to the care team/LIP, patient’s condition, response to medications and/or treatments and any significant findings/incidents (actual/potential – Risk Assessment).
  • Conducts patient education including chronic disease management (Diabetes, Hypertension, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), fall/injury prevention, basic nutrition, post-operative wound monitoring and care.
  • Administers care consistent with program policies, standard operating procedures and LIP orders.
  • Coordinates appointments, additional home health visits  and follow-up with providers and facilities as appropriate/ordered.
  • Provides both Basic and Advanced Life Support within their scope and capabilities (equipment limitations) in collaboration with local EMS agencies. 
  • Perform specialty visits (such as HNI high risk pilot).
  • Completes full scope of stat medic duties.
  • Performs daily equipment checks. 
  • Orders equipment and supplies.
  • Responsible for POC lab testing device, other equipment, and all compliance documentation.
  • Assists Medical Director with quality assurance activities – chart reviews, documentation review, peer review, QA presentations.
  • Assists with day-to-day field training, chart reviews, and documentation improvement, especially for new hires.
  • Oversight responsibility of all daily medic assignments.
  • Responsible for education and onboarding of new paramedics and for maintaining the Onboarding Manual.
  • Provides regular coaching and education to the paramedic team, including CEU’s.
  • Participates in the hiring process.
  • Assists with the Annual Performance Review process.
  • Assists the Medical Director and Department Administrator with business development.
  • Represents the Community Paramedicine team widely within the Enterprise and at national meetings.
  • Actively involved in research activities of the Community Paramedicine team.
  • Serves as Program Manager for department by developing, implementing and maintaining all projects/program.
  • Manages multiple projects and priorities with competing deadlines.
  • Provides direct supervision for the Community Paramedics on the team.
  • Responsible for guiding and leading the team to ensure department objectives are met.
  • Responsible for ensuring the team is meeting all job expectations.
  • Handles performance issues as appropriate.
  • Responsible for completing annual performance appraisals for the team, obtaining input as appropriate from key stakeholders.

Education:

  • Completion of accredited paramedic training program (EMT-P). 
  • Bachelor’s degree in Business Administration or Healthcare Administration preferred.

Certifications:

  • Current certification as an Emergency Medical Technician – Paramedic in the Status of practice.
  • Basic Cardiac Life Support (BLS) through the American Heart Association (AHA).
  • Advanced Cardiac Life Support (ACLS) through the American Heart Association (AHA).
  • Valid Ohio Driver’s license upon hire; per policy 8-175 thereafter.
  • Must have reliable transportation and proof of automobile insurance coverage with $100,000/$300,000 coverage, upon hire; per policy 8-175 thereafter.
  • Meets all STAT medic requirements (education and onboarding).
  • Community Paramedicine Certification (CP-C) within 6 months of Lead Paramedic appointment.
  • EMS-Instructor credentialed, or obtains within six months of hire.
  • Completes formal coach training in Cleveland Clinic model.

Competencies:

  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
  • Requires ability to work collaboratively with healthcare professionals and family/caregivers to ensure “Patients First” philosophy and achieve shared goals.
  • Must be able to work in a residential / community environment and take appropriate action.
  • Position requires travel and driving.
  • Position is outside the normal “hospital” environment and requires working in patient’s homes/care areas.
  • Advanced pharmacology and EM knowledge base and training in acute and chronic care management.
  • High level communication skills and judgment. 
  • Self-starter and reliable.
  • Formal coaching training from an internal program (GLLI) or external (Weatherhead, etc.)

Work Experience:

  • At least two years of clinical experience as an EMT-P.

Physical Requirements:

  • Requires full body motion to assist patients and practice within a home environment.
  • Ability to move and lift patients, manual finger dexterity with good eye-hand coordination, involves extensive standing and walking. 
  • MEDIUM WORK – Exerting 20-50 pounds of force occasionally, and/or 10-25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
  • Physical demand requirements are in excess of those for “Light Work.”

  • Follows standard precautions using personal protective equipment as required.
